On November 1, 2017, SOMOS Community Care officially launched its reorganization into Practice Transformation teams. These multi-disciplinary teams, including Community Health Workers, Physician Engagement Specialists, and Project Managers, lead our efforts to support our network of physician practices to prepare for success in the emerging healthcare environment, which focuses on Population Health Management and Value-Based Care and Payment.
We are making a significant investment in building the skills of our teams, both for the benefit of our physician network and the benefit of our staff members. These teams represent our commitment to the long-term success of our physician partners as DSRIP shifts its focus to the VBP objectives which were embedded in the Waiver which created both programs.
Less than two months after SOMOS officially launched a reorganization into the teams, our teams are off to a terrific start, and are making more progress every day. Thanks to our dynamic EVPs of Operations (Lidia Virgil and Gloria Wong) and our resourceful segment leads (VPs Jimmy Ha, Shariff de los Santos, and Soraya Sussman), our teams have been incredibly productive in serving our practices. We look forward to greater success in 2018, when our networks will be put to the test by the expanding Pay for Performance focus of DSRIP.
