To get help from SOMOS’ special healthcare program for things like food, housing, and transportation, you need to be a Medicaid member and have some specific needs. These needs might include things like having a serious health condition, being at risk of losing your home, or recently getting out of jail or the hospital. 
 
SOMOS network providers and partners will check to see if you qualify, so they can connect you with the right kind of help to get you back on track, focused on your health, and living your best life!
