Did you know there’s someone on your Medicaid team who can help with more than just medical bills? They’re called Social Care Navigators, and they’re here to connect you with resources that can improve your overall health and well-being. Here’s how:
We listen and understand
Your navigator will talk with you to see if you’re facing any challenges outside of doctor’s visits. This could be anything from finding affordable housing to getting healthy food on the table or having reliable transportation to appointments.
We connect you to resources
Once we understand your needs, we’ll work to find the right programs and services in your community that can help. This might include food banks, job training programs, or even help with childcare.
We work with your doctor
From coordinating social services with your medical treatment plan, to connecting with your doctor, our navigator will make sure you get the best possible care.
We fight for you
Navigator’s goal is to make sure you stay healthy and independent, therefore they advocate for you if you’re facing any difficulties accessing the services you need.
