Value-Based Care

SOMOS’ success is a story of transformation, growth, and ingenuity. It is a story of what good government policies can achieve when the government works with those who are in the know. It is the story of transformation from the Delivery System Reform Incentive Payment (DSRIP) Program that funded initiatives focused on reducing avoidable hospital use while promoting an integrated, coordinated, and value-based plan of care.

A Value-Based Payment (VBP) system rewards network providers for providing extraordinary care to vulnerable patients. Value-based care ties the amount health care providers earn for their services to the results they deliver for their patients, such as the quality, equity, and cost of care. Through financial incentives and other methods, value-based care programs aim to hold providers more accountable for improving patient outcomes while also giving them greater flexibility to deliver the right care at the right time.

Value-based care covers medical and behavioral needs and the myriad social determinants that may impact a person’s well-being and opportunities. These factors are known as the Social Determinants of Health and are factors that you, as SOMOS network providers, have focused on since our inception. Your comprehensive, holistic grasp of your patient’s needs and your knowledge of who and where they come from has transformed healthcare in New York.
